Service Request Form
PLEASE READ BEFORE SUBMITTING THIS FORM
For approved Warranty Service, include the following in the box:
- A copy of your proof of purchase from an Authorized PSC Motorsports Dealer
- This completed Service Request Form
- Your issued RGA number, written clearly on the outside of the package
We recommend shipping with UPS, FedEx, or DHL, and insuring the package for full replacement value. If you choose a different carrier, that shipment is sent at your own risk.
PSC Motorsports isn’t responsible for damage that happens during shipping. Any claims for shipping damage will need to be filed directly with the carrier.
Before shipping your item(s) please take note of the following:
- Completely drain all fluid from the item. If you have port plugs, use them.
- Wrap the part in plastic or place it in a sealed bag to prevent leaks.
If the box shows signs of fluid leaks, UPS, FedEx, or DHL may label it as hazardous material, which can delay or even stop the shipment.
Turnaround Time
Typical turnaround is 10–15 business days once we receive your item.
If a product is found to be defective, PSC will repair it, replace it, or issue credit within 30 days of receipt.
